• 100 ml/50ml

• Order Monthly Refills as needed
• You can order refills for one year or 6 bottles  which ever comes first and then by law you are required to have another TeleDoc Checkup.

STEP 2:  Fill out the Patient Order Form 

• Please fill out every line in order to submit then you will have to verify your email. 
• Any questions you don't know put NA (Not Available) on the line. 
• DO NOT fill out the Credit Card section just put NA on the lines.  The Credit Card section is for auto- monthly patients.  
• You will have already purchased in the skin care shop.
• After you fill out the medical form please text me photos of the area we are treating.
